These Corporate Wellness Program Code Terms (these “Corporate Wellness Code Terms”) supplement the ByHeart Terms of Service (the “Terms of Service”) and apply to you if you purchase ByHeart products or subscriptions with two (2) codes (“Code(s)”) offered by an employer providing or sponsoring the Codes on your behalf (“Member”). These Corporate Wellness Code Terms become effective on the date you first accept, redeem or receive the Code(s) and continue up to the date on which you cease to be eligible to receive the Code(s) (the “Code End Date”).

The Code(s) will be available to you: (i) for as long as there is an agreement in place between the Member and ByHeart; (ii) you meet the eligibility requirements for the Code(s); (iii) you comply with these Corporate Wellness Code Terms; and (iv) until any stated expiration date. 

Member may make available to you the Codes that are valid for either: 

• A one-time fifty percent (50%) discount off your first order of $78 or more of ByHeart products; or
• A ten percent (10%) discount off of the first twelve (12) recurring orders of any ByHeart subscription.

The Codes are only valid on orders placed through the ByHeart website using the email address associated with your employer and used to create a ByHeart account.  Limit two (2) Codes per eligible customer and two (2) Codes per mailing address.  A ByHeart account is required to redeem the offer.  You are responsible for taxes, shipping, and the balance of the order cost.  A valid credit card is required to create a new account. Code is not valid on any third-party websites.  Products will only ship to addresses in the 50 United States and DC. No military addresses. Code cannot be combined with any other offer, coupon, discount, or promotion.  The Codes are personal to you and may not be transferred, bartered, or sold. Any attempt to use Codes in violation of these Corporate Wellness Code Terms. Codes have no cash value and are not property. 

The Codes must be used on two (2) separate orders and cannot be combined for one (1) order. If you use the Code on a subscription and pause or cancel the subscription before all twelve (12) orders have been fulfilled, you will forfeit the discount. 

If the agreement between Member and ByHeart terminates or you cease to be employed by Member, you will no longer be eligible to redeem the Code.
